Health outcomes are shaped not only by genetics and clinical care but by the broader social and economic conditions in which people live. Education, employment, housing, and income — collectively known as the wider determinants of health — have a profound and lasting impact on individual and population wellbeing.

These inequalities  are not evenly distributed. They intersect strongly with ethnicity, geography, disability, and gender — meaning that some communities experience a persistent cycle of disadvantage and poor health across generations.

We are committed to actively addressing the wider determinants of health through our operating model and through every contract we deliver.

This approach aligns with NHS England’s Core20PLUS5 framework, which explicitly calls for action on wider determinants through collaboration and community engagement. It also supports the Social Value Model used in NHS and local government procurement, which rewards providers that contribute to local economic regeneration, fair work, and wellbeing beyond the contract itself.
